An 84-year-old man has a painless lesion on his cheek which won’t heal. He thought he’d traumatised a spot, but it has persisted for more than a year, slowly getting larger. There is no pain. On examination he has on his cheek a 0.5cm papule with slight central ulceration. What's the cause?
